Bring with you: ID or birth certificate (children from 0 to 15 years old).

— Certificate of temporary disability of the student (form No. 037/у)

If the patient has an acute illness.

When examining persons who have received an occupational disease – information from the health care organization carrying out the examination in establishing the diagnosis of occupational disease and poisoning (determining the connection of the disease with the profession).

— Certificate of health (form 027/у)

If the patient is healthy and is not registered with chronic diseases, in this case the doctor will issue a certificate immediately at the appointment. After receiving the results of the examination, the patient comes for a second appointment, and the doctor gives a conclusion.
